If your child takes regular PRESCRIBED MEDICATION AT HOME please click on the link below and complete the Questionnaire so that we can record it in Compass.  It is important that you let us know of any medication your child regularly takes at home in case of an emergency.  If your child needs to be transported to hospital by Ambulance this is a question that the Paramedics ask and if we do not have this on record we are unable to inform them of it.
